Welcome to The Well Counseling Practice, PLLC ("Practice," "we," "our," or "us"). These Terms of Service ("Terms") govern your use of our website (www.thewellcounselingpractice.com) and our counseling services. By accessing or using our website or services, you agree to these Terms.
If you do not agree with these Terms, please do not use our website or services.
1. Services Provided
The Well Counseling Practice, PLLC offers individual and couples counseling, both in person and virtually. Services are available only to residents of Illinois and Wisconsin, as we are licensed to practice in these states.
We do not provide emergency mental health services. If you are in crisis, please call 911, 988 for mental health emergencies, or visit your nearest emergency room.
2. Eligibility
You must be at least 18 years old to use our services unless a parent or legal guardian provides consent for minors. By using our website and services, you confirm that you meet these eligibility requirements.
3. Appointments & Cancellations
• Sessions are 50-55 minutes unless otherwise agreed upon.
• Clients must cancel or reschedule appointments at least 24 hours in advance through the SimplePractice client portal or by calling/texting (224) 764-1949.
• If you fail to cancel within 24 hours or do not attend a scheduled session, you will be charged $100.
• Late arrivals may result in a shortened session without fee adjustment.
4. Fees & Payment
• Accepted payment methods: credit card, check, insurance, HSA, FSA.
• A $10.00 fee will be charged for any returned checks.
• Payment is due at the time of service unless prior arrangements have been made.
• Any unpaid balances may result in service suspension.
5. Confidentiality & Privacy
• Your privacy is important. Please review our Privacy Policy for details on how we protect your information.
• Electronic communications (emails, texts) outside of our HIPAA-compliant iPlum messaging system and SimplePractice Client Portal are not guaranteed to be secure.
• We do not share client information without explicit consent, except as required by law.
6. Electronic Communication & Telehealth
• Telehealth services are available but may have limitations compared to in-person sessions.
• We use HIPAA-compliant technology for telehealth. However, we cannot guarantee absolute security for all digital communications.
• If using email or text for scheduling, please do not include sensitive information.
7. Social Media & Professional Boundaries
• We do not accept friend or contact requests from current or former clients on social media.
• Public interactions (e.g., liking or commenting on our content) may impact confidentiality.
8. Termination of Services
• Either party may terminate the therapeutic relationship at any time.
• If therapy is not effectively utilized, or if payment issues arise, we may terminate services with appropriate notice.
• Failure to schedule an appointment for three consecutive weeks without prior arrangements may result in service termination.
• If therapy is terminated, we will provide referral options upon request.
9. Limitation of Liability
• The Well Counseling Practice, PLLC does not guarantee specific results from therapy.
• We are not liable for any damages resulting from website use, telehealth limitations, or external factors beyond our control.
10. External Links & Third-Party Services
Our website may contain links to third-party resources. We are not responsible for the content, security, or practices of those sites.
11. Governing Law & Dispute Resolution
These Terms are governed by the laws of the State of Illinois. Any disputes shall be resolved through mediation or arbitration before legal action is pursued.
